Program Manager - YMSM Mobile
Role summary
Community Healthcare Network (CHN) is seeking a Program Manager for its YMSM Mobile program. This role oversees the daily operations of HIV prevention, PrEP/PEP requests, and STI screening services, with a focus on young men who have sex with men (YMSM). Responsibilities include managing outreach efforts, ensuring medication adherence, training and supervising staff, developing partnerships, maintaining data entry in AIRS and EMR systems, and leading quality improvement initiatives. The position requires an Associate Degree or equivalent experience, knowledge of the Queens area, and extensive understanding of HIV/AIDS, PEP/PrEP, and STI follow-up.

POSITION SUMMARY:
Oversees the day-to-day activities of the HIV prevention program and linkage to care, PrEP and PEP requests, STI screening and prevention education and maintains community engagement. Manages and supervises outreach efforts directed towards young men who have sex with men (YMSM) who are at risk for STIs and HIV, including helping to ensure medication adherence.
DUTIES AND RESPONSIBILITIES:
- Oversees the daily operations of the YMSM program at all the different sites
- Trains and supervises the community counselor role
- Informs staff about agency updates, health issue updates, and other work-related information.
- Identifies new locations/partnerships where the YAP can increase outreach activities and client engagement
- Identifies and facilitates new linkage agreements with organizations to increase client enrollment
- Evaluates site productivity periodically
- Accompanies community counselors to various outreach activities
- Develops and maintains referral network and assists in the referral process
- Develops Policy and Procedure Manual for YMSM as well as other departmental policies as appropriate
- Oversees the planning of program schedules, scheduling of participants, preparing and distributing program announcements, preparing written program materials, and evaluating programs’ effectiveness
- Maintains timely data entry on AIRS and monthly reports
- Ensures timely data entry in electronic medical records systems by self and staff
- Develops reports for funders and CHN outlining intervention, quality assurance and evaluation activities
- Oversees and leads QI initiatives as required by the AIDS Institute and CHN
- Effectively recommends or participates in hiring, discipline or termination
- Facilitates related services for health center patients and CHN staff as appropriate, with respect to their confidentiality and privacy

EDUCATION & EXPERIENCE:
- - Associate Degree from an accredited college plus a minimum of two (2) years’ work experience or the equivalent combination of education and work experience
- Experience working in and knowledgeable about the Queens area as new locations may need to be identified
- Extensive knowledge of HIV/AIDS issues, including current treatments., PEP/PrEP and STI follow-up
- Prior involvement in research projects preferred
