DayaMed is seeking an experienced Nurse Practitioner to serve as a Clinical Program Manager for Virtual Care. This role focuses on designing, implementing, and optimizing technology-enabled care programs such as Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and telehealth services. The ideal candidate will be a clinical subject matter expert with at least 5 years of clinical experience and a strong understanding of virtual care programs, CMS requirements, and healthcare economics. Responsibilities include developing clinical protocols, evaluating program financial performance, ensuring regulatory compliance, and collaborating with product and engineering teams to integrate clinical workflows into technology platforms. This is a remote, full-time position.

Position Overview

DayaMed is seeking an experienced Nurse Practitioner to serve as the Clinical Program Manager for Virtual Care, responsible for helping design, implement, and optimize clinical programs delivered through technology-enabled care models.

This role will focus on the development and continuous improvement of programs including Remote Patient Monitoring (RPM), Chronic Care Management (CCM), Behavioral Health Integration (BHI), Transitional Care Management (TCM), and telehealth services.

The Clinical Program Lead will serve as a clinical subject matter expert working closely with leadership, operations, product, and engineering teams to ensure care delivery aligns with clinical best practices, regulatory requirements, payer expectations, and sustainable program economics.

This role works in partnership with the Clinical Supervisor to expand and optimize DayaMed’s virtual care programs while supporting the strategic growth of the organization’s technology-enabled care delivery model.

The ideal candidate brings deep experience operating or building virtual care programs and understands how clinical protocols, documentation standards, and utilization patterns impact patient outcomes, compliance, reimbursement, and overall program sustainability.

Key ResponsibilitiesVirtual Care Program Development

  • Lead the development and refinement of virtual care programs including RPM, CCM, BHI, TCM, and telehealth services
  • Establish standardized clinical protocols and care pathways for chronic disease management
  • Design scalable workflows that support high-quality virtual care delivery
  • Partner with the Clinical Supervisor to ensure alignment between clinical operations and program strategy

Program Economics & Sustainability

  • Evaluate financial performance of virtual care programs including reimbursement performance, utilization patterns, and cost efficiency
  • Identify opportunities to improve margin and sustainability across RPM, CCM, BHI, and other CMS programs
  • Collaborate with finance and operations teams to ensure program design supports both strong clinical outcomes and economic performance
  • Provide leadership with insights and recommendations related to program scalability and ROI

Regulatory Strategy & CMS Innovation

  • Monitor emerging CMS programs, policy updates, and innovation models related to virtual care delivery
  • Evaluate opportunities to align DayaMed programs with evolving CMS initiatives and outcome-based reimbursement models
  • Ensure program design supports regulatory compliance, documentation standards, and value-based care reporting requirements

Clinical Protocols & Quality Initiatives

  • Develop and maintain evidence-based clinical protocols aligned with evolving clinical guidelines
  • Serve as a subject matter expert on healthcare quality programs such as STAR Ratings and value-based care metrics
  • Collaborate with analytics and operations teams to align clinical workflows with quality reporting and performance improvement initiatives
  • Identify opportunities where RPM, CCM, and care management programs can improve quality scores and patient outcomes

Workflow Optimization & Population Health

  • Evaluate clinical operations to identify opportunities to improve efficiency, care coordination, and patient engagement
  • Develop standardized documentation practices and care planning frameworks
  • Design escalation pathways and risk stratification models for complex or high-risk patients
  • Support development of reporting frameworks that measure program outcomes and patient engagement

Technology & Clinical Innovation

DayaMed operates as a technology-enabled healthcare organization, and this role plays a key part in integrating clinical expertise into platform development.

Responsibilities include:

  • Partnering with product and engineering teams to integrate clinical workflows into technology platforms
  • Providing clinical input into the development of AI-enabled tools supporting care management and patient monitoring
  • Ensuring technology solutions align with clinical best practices, compliance requirements, and scalable care delivery models

Qualifications

Required

  • Active Nurse Practitioner license (multi-state preferred)
  • 5+ years clinical experience
  • Experience working with one or more of the following programs:
  • Remote Patient Monitoring (RPM)
  • Chronic Care Management (CCM)
  • Behavioral Health Integration (BHI)
  • Transitional Care Management (TCM)
  • Experience delivering or managing telehealth-based care
  • Strong understanding of clinical protocols, care pathways, and population health management
  • Familiarity with CMS documentation and billing requirements for virtual care programs

Preferred

  • Experience designing or launching virtual care programs
  • Knowledge of value-based care models and healthcare quality frameworks
  • Experience working within technology-enabled healthcare platforms
  • Experience collaborating with product, engineering, or analytics teams
  • Background in population health or care management strategy

Pay: $100,000.00 - $125,000.00 per year
