Peninsula Healthcare Connection (PHC) is seeking an Aging Services Senior Program Manager to be the agency specialist for aging services. This role will ensure high-quality services for older adults in New Directions programs and will lead the service provision, oversight, and growth of the Ability Link program. The position combines program leadership, direct client care, and the development of a private-pay senior care coordination program. PHC is a non-profit healthcare agency dedicated to providing medical, behavioral health, and social services to underserved populations.

Summary
Peninsula Healthcare Connection is currently recruiting an Aging Services Senior Program Manager who will serve as the agency specialist on aging services. This role will ensure high quality services for aging adults in New Directions programs, and will have primary responsibility for service provision, oversight, and growth of the Ability Link program. This role combines program leadership, direct client care, and the development and growth of our private-pay senior care coordination program.

Aging Services Senior Program Manager Duties Will Include:
- Supporting program development, oversight, and evaluation for all New Directions programs that serve older adults; including development and implementation of new programs and contracts.
- Support New Directions direct service staff with community-based services as needed.
- Overall responsibility for program development, service provision, and growth of Ability Link.
- Provide direct services to Ability Link clients including intake, assessment, creation of care plan, and ongoing care coordination.
- Build and maintain a trusted network of community partners, service providers, and referral relationships.
- Assist with recruitment, onboarding, and management of Ability Link and New Directions staff.
- Support marketing and development efforts in collaboration with Executive Leadership.
Aging Services Senior Program Manager Requirements:
- Masters Degree in Gerontology, Social Work, Nursing, or related field preferred; Bachelors degree and relevant experience required
- Minimum two years’ experience serving older adults and/or individuals with complex medical, behavioral health, or social needs.
- Demonstrated ability to build collaborative partnerships with clients, families, providers, and community members, including individuals from diverse cultural and socio-economic backgrounds.
- Valid CA driver’s license, reliable transportation, and legally required insurance.
Aging Services Senior Program Manager Qualifications and Skills:
- Knowledge of principles, ethics, and best practices of trauma informed case management, care coordination, and older adult services.
- Experience with transitions from institutional settings to community living.
- Knowledge of aging services, long-term care systems, Medicaid/Medicare, and community-based supports.
- High self-management and reliability: excellent organization, follow-through, flexibility and time management with minimal supervision.
- Strong communication skills (oral and written), including the ability to explain complex systems in plain language.
Pay: $95,000.00 - $110,000.00 per year
