Role summary
Phoenix Hospice is seeking compassionate Registered Nurse (RN) Case Managers for their team in Kansas City, MO & Surrounding Areas. This hybrid role involves providing direct hospice care, developing patient-centered care plans, and collaborating with an interdisciplinary team. The ideal candidate possesses strong managerial, organizational, and interpersonal skills, with a focus on patient and family support. A Kansas or Compact RN license, valid driver's license, and auto insurance are required. Hospice experience is preferred but training is provided. - Pay Range: $80,000 - $90,000
- $5000 Sign On Bonus
Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. Will work with the interdisciplinary team to develop and implement a plan of care that is patient/family centered, with goals of care discussions and guidance with healthcare decisions to give quality of life.
Our ideal candidate should possess a high level of energy and motivation, coupled with strong managerial, organizational, and interpersonal skills. They must be capable of providing precise and effective guidance and support, even in demanding situations, in order to enhance learning opportunities and foster professional growth.
Requirements- Active Kansas or Compact RN license
- Hospice experience preferred; training provided
- Valid driver’s license and auto insurance
- $5,000 Sign-On Bonus
