Role summary

This Mental Health Case Manager role supports patients and families in managing mental health and substance use concerns. Responsibilities include discharge planning, resource navigation, community engagement, and acting as a liaison between patients, families, providers, and community resources. The role also involves monitoring insurance benefits, assisting with appeals, participating in involuntary commitment processes, and ensuring timely care and disposition. The position requires the ability to work independently, manage time effectively, and demonstrate adaptability and conflict resolution skills. A Master's Degree in a relevant field is required, with no prior experience necessary.

Overview

Supports patients and families in managing mental health and substance use concerns, assisting with discharge planning, resource navigation, and community engagement. Serves as a resource and placement specialist, ensuring timely referrals, warm handoffs, and effective communication. Provide education, advocacy, and support to enhance patient self-management and recovery. Facilitates inpatient and outpatient connections, monitors placements, conducts safety assessments, arranges transportation, and addresses social determinants of health. Acting as a liaison between patients, families, providers, and community resources. Proactively works to prevent delays in care and disposition. Reviews patient admissions and continued stays for appropriateness, monitors insurance benefits, and ensures care aligns with coverage to optimize payment. Assists with insurance appeals and utilization reviews. Supports clinical staff with coverage guidance and medical record reviews. Participates in the involuntary commitment process, including case tracking and representing the hospital in hearings. Must be able to work independently, prioritize tasks, and manage time efficiently. Demonstrates a flexibility in accepting diverse work assignments, managing stress related to change, and resolving conflicts effectively.

Mental Health & Well-Being Hospital – WakeBrook provides innovative solutions to address the mental health issues among adults in our community.

WakeBrook will serve adults in need of inpatient care for psychiatric illnesses. This is an exciting opportunity to be part of a critical service for our community and care for adults who need our help.

WakeMed will also open a 150-bed Mental Health & Well-Being hospital in Garner, NC in 2029, which will offer additional opportunities when it is close to completion.
