Care Continuity is seeking a Platform Product Manager to own the success of ED Connect and Readmission IQ. This individual contributor role focuses on improving patient engagement, streamlining coordinator workflows, and ensuring platform reliability. Role Overview

As Platform Product Manager, you will own the success of two critical product areas: ED Connect and Readmission IQ, serving care coordinators and the patients they support. You'll partner closely with engineering and analytics teams to improve patient engagement, streamline coordinator workflows, and ensure our platform reliably supports high-volume patient navigation programs.

This is a high-impact individual contributor role focused on execution, iteration, and measurable outcomes. You'll drive the product development lifecycle (PDLC) for your product areas while collaborating with design, engineering, analytics, and operations teams to continuously improve the patient and coordinator experience.

You'll also play an important role in identifying opportunities to improve operational efficiency across our platform. This includes evaluating new technologies, automation approaches, and potential vendor partnerships that help Care Continuity scale its patient navigation capabilities.

This role is ideal for a product manager who enjoys improving real-world workflows while exploring new technologies that can help teams work smarter and scale impact.

What You'll Do

  • Own product roadmap and execution for ED Connect and Readmission IQ
  • Define and prioritize improvements to coordinator workflows and patient engagement experiences
  • Partner with engineering to deliver reliable, scalable platform features that support patient navigation operations
  • Collaborate with design and analytics teams to identify opportunities to improve patient communication and engagement
  • Translate user needs and operational insights into clear product requirements for engineering and analytics teams
  • Lead end-to-end execution across discovery, design, development, testing, and launch
  • Work closely with customer success and operations teams to understand real-world workflow challenges and opportunities
  • Identify opportunities to improve operational efficiency through automation, workflow improvements, and emerging technologies
  • Evaluate new tools, vendors, and AI-driven capabilities that could enhance Care Continuity's platform and operational efficiency
  • Measure product success using engagement metrics, workflow efficiency, and program outcomes

What We're Looking For

  • Experience delivering software products in collaboration with engineering teams
  • Strong ability to translate user needs and operational challenges into clear product requirements
  • Experience working with data-rich products or operational platforms
  • Curiosity about emerging technologies such as automation platforms, AI-assisted workflows, or agentic systems
  • Ability to evaluate new technologies and vendor solutions through a product and operational lens
  • Excellent collaboration and communication skills across technical and non-technical teams
  • Comfort operating in fast-moving environments with evolving priorities
  • Strong bias for action and focus on delivering practical solutions
  • Interest in improving healthcare delivery through thoughtful product design

Nice to Have

  • Experience building products for healthcare providers, care coordination teams, or patient engagement platforms
  • Familiarity with messaging platforms, workflow automation, or engagement tools
  • Experience supporting A/B testing or product experimentation
  • Exposure to AI-assisted workflows, agentic AI tools, or automation platforms
  • Background in health tech, healthcare operations, or digital health startups
  • Experience working in early-stage or growth-stage companies

Salary & Benefits

  • Salary range: $120,000 - $150,000 annually, depending on experience and location
