Role summary
Comunilife is seeking experienced Case Managers for their HRA Scatter Program in the Bronx. This full-time, onsite position involves providing case management services to approximately 22 clients, including completing comprehensive assessments, service plans, and progress notes. Responsibilities include assessing client needs, making referrals, providing psychoeducation, and collaborating with interdisciplinary teams. A Bachelor's degree in human services with two years of experience is preferred, with alternative qualifications considered. Experience with specific populations and knowledge of HRA guidelines are beneficial. Proficiency in Google Workspace and Microsoft Office Suite is required.

The Case Manager’s main responsibility is to provide case management services for a caseload of approximately 22 clients. The Case Manager prepares comprehensive assessments and service plans. Home visits and Fieldwork are required.
