Engage Senior Therapy is seeking a Per Diem Physical Therapist for as-needed coverage in Assisted Living Facilities in Concord, NH. The role involves conducting evaluations, developing and implementing treatment plans, and delivering interventions to improve mobility, reduce pain, and restore function in a geriatric population. Responsibilities include fall prevention, patient education, documentation using EMR systems (Prompt EMR), and effective communication with residents, families, and staff. Engage Senior Therapy is seeking a dedicated Per Diem Physical Therapist to join our team, providing as-needed coverage at Assisted Living Facilities throughout our Concord, NH region.

About the Role

As a Per Diem Physical Therapist at Engage, you'll provide individualized, evidence-based care that helps older adults improve mobility, maintain independence, and enhance their overall quality of life. You'll work with a diverse geriatric population across independent living, assisted living, dementia, and memory care settings. This is an as-needed, on-call opportunity ideal for a clinician looking for scheduling flexibility and the ability to pick up additional hours based on patient needs and facility coverage requirements.

Your day-to-day:

  • Conduct comprehensive evaluations to assess strength, balance, mobility, pain, and functional impairments.
  • Develop and implement individualized treatment plans focused on improving mobility, reducing pain, and restoring function.
  • Deliver hands-on interventions including therapeutic exercise, balance training, gait training, neuromuscular re-education, and manual therapy techniques.
  • Promote resident safety and fall prevention through functional assessments, education, and preventative interventions.
  • Provide coverage across one or more assisted living communities based on patient needs and scheduling availability.
  • Educate residents and caregivers on home exercise programs and strategies to support long-term independence and wellness.
  • Maintain timely, accurate documentation and monitor patient progress using EMR systems in compliance with state and facility standards.
  • Communicate effectively with residents, families, facility staff, and the broader Engage clinical team to ensure coordinated, patient-centered care.

    • What You Bring

  • Graduate of a CAPTE-accredited Physical Therapy program.
  • Active PT license in good standing (or eligibility for licensure).
  • Passion for delivering compassionate, evidence-based patient care to a geriatric population.
  • Strong communication and interpersonal skills with a collaborative, team-oriented mindset.
  • Ability to work independently while staying connected with a supportive clinical team.
  • Flexibility and willingness to provide coverage based on patient and facility needs.
