Role summary
Expedient Home Health Care is seeking a compassionate and skilled Bilingual RN or LVN to join their Home Health Team. The role involves providing comprehensive care to patients in their homes, including assessments, developing care plans, patient education, wound care, and clinical direction to aides and LPNs/LVNs. Responsibilities also include completing documentation such as OASIS assessments and progress notes, and communicating with physicians and the interdisciplinary team. A current RN and/or LVN license, a driver's license, and one year of home health experience are required. Bilingual skills are preferred.

Job Description
Looking for a compassionate and skilled nurse to join our team. You'll play a key role in providing comprehensive care for our patients.
Your responsibilities will include
- Conducting initial and ongoing assessments of patients' physical, functional, and psychosocial needs in their place of residence.
- Developing and implementing personalized care plans to ensure quality and continuity of care.
- Educating patients and their families on disease processes, self-care, and providing wound care and treatments as ordered.
- Ensuring patients meet the criteria for home health
- Providing clinical direction to home health aides and LPNs/LVNs.
- Completing all necessary documentation, including OASIS assessments and progress notes, to track patient progress and ensure a high standard of care.
- Communicating regularly with physicians and the interdisciplinary team about patient needs and changes.
Qualifications
EXPERIENCE AND REQUIRED SKILLS
Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Licensure: Current state license as a registered nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact. Current driver’s license.
Experience: Experience as a RN in a Home Health Setting.
Skills: Bilingual Preferred. Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Must have one year home health experience. Must read, write and comprehend English.
