Built for nurses who want autonomy, not isolation.

At Health Link Home Health & Hospice, we believe great patient care starts with supporting the clinicians who deliver it. For over 20 years, we’ve served the Bay Area with a focus on clinical quality, strong systems, and a collaborative culture. We’re growing our San Francisco team and looking for a compassionate, organized RN Case Manager.

Whether you’re a seasoned home health nurse or a hospital nurse looking for a change of pace, you’ll get the independence of home health with a full team behind you.

Why SF Nurses Choose Health Link

  • A full-time paycheck, every week. You’re paid hourly for your scheduled hours, including drive time and documentation. No per-visit pay, no unpaid charting, and a growing census that keeps your caseload steady.
  • When you clock out, you’re off. No after-hours patient calls. Our dedicated after-hours nurse handles calls once your day ends, so your evenings belong to you and your family.
  • A full support team behind you. A dedicated scheduling team confirms visits with your patients, so you’re not left waiting on callbacks. Our orders department handles physician signatures and follow-up, QA reviews your OASIS after submission, and clinical leadership and intake are available in real time when you need help.
  • Less paperwork, more patient care. AI-assisted charting on your company-issued iPad cuts documentation time so you can focus on your patients.
  • Parking and mileage covered. Full-time nurses get a company credit card for parking, so there’s no paying out of pocket and waiting for reimbursement. Mileage is reimbursed, and a company vehicle may be available.
  • A manageable pace. Organized scheduling, flexible start times, and a team that respects your time.
