Role summary

Home Sweet Home is seeking a compassionate Occupational Therapist (OT) to join their growing Home Health team in Maricopa County. This Per Diem (PRN) role offers flexible scheduling and focused routes to support work-life balance. The OT will provide one-on-one care in patients' homes, conducting evaluations, developing treatment plans, recommending adaptive equipment, and educating patients and caregivers to improve independence and quality of life. Collaboration with a multidisciplinary team and accurate EMR documentation are key responsibilities. A current Arizona OT license and at least one year of experience are required.

Occupational Therapist Responsibilities

As a Home Health Occupational Therapist, you will:

Complete in-home Occupational Therapy evaluations and reassessments

Develop individualized treatment plans to improve independence with Activities of Daily Living (ADLs)

Provide rehabilitation for patients recovering from illness, injury, surgery, or hospitalization

Recommend adaptive equipment, home modifications, and fall prevention strategies

Educate patients and caregivers on safety, mobility, and functional independence

Collaborate with physicians, nurses, Physical Therapists (PT), Speech-Language Pathologists (SLP), and the interdisciplinary care team

Document patient progress accurately using EMR while maintaining Medicare and agency compliance

Qualifications

Current Arizona Occupational Therapist (OT) license

Graduate of an ACOTE-accredited Occupational Therapy program

Minimum one year of Occupational Therapy experience (Home Health preferred)

Strong communication, organization, and time management skills

Experience with EMR documentation

Valid driver's license, reliable transportation, and current auto insurance
