Role summary

Home Sweet Home is seeking a licensed Physical Therapist (PT) for home health services in the North Phoenix area. This role involves providing one-on-one care in patients' homes to improve their strength, mobility, and independence. Responsibilities include conducting evaluations, developing treatment plans, educating patients and caregivers, and documenting visits using WellSky Kinnser EMR. The position offers flexible scheduling, competitive per-visit compensation, and a supportive team environment. A minimum of 1 year of clinical experience is required, with home health experience preferred. Active Arizona PT license and CPR certification are mandatory.

Location: North Phoenix areaCompensation: Per Visit | Earn up to $103,500 annually based on productivity

Sign on Bonus for full time: $5,000

Schedule: Per Diem, Part Time, Full Time

Your Role: Home Health Physical Therapist

You will provide one-on-one care in patients’ homes, helping them regain strength, mobility, and confidence in their daily lives.

Key Responsibilities

Conduct in-home evaluations, start of care, and follow-up visits

Develop and implement individualized treatment plans

Improve strength, balance, mobility, and functional independence

Educate patients and caregivers on safety and equipment use

Document visits using WellSky Kinnser EMR

Collaborate with physicians, RNs, and the care team

Qualifications

Active Arizona Physical Therapist license

CPR certification (AHA)

Minimum 1 year of clinical experience (home health preferred)

Strong clinical judgment and time management

Reliable transportation, valid driver’s license, and insurance

Strong communication and a patient-first approach
