About Us:
Main Street Health is the nation's largest provider of value-based care exclusively serving rural America. We believe in the old ways of medicine when healthcare was simpler. By partnering with rural primary care doctors, we reinforce the importance of trust and relationship-driven care in rural communities. We provide our partners with the staff, technology, and processes necessary to succeed in a value-based delivery model.
About the Role: Navigating the changes to our healthcare system can be intimidating and confusing for doctors and patients alike. The Health Navigator’s role is to make it easier. You will start by developing trusted relationships with our patients and providers and will assist with various patient-facing, administrative, and logistical tasks so that doctors can focus on delivering high-quality medical care to their patients. Navigators work for Main Street but go to work every day inside one of our partner clinics alongside clinic staff. They meet with and call patients, enter data into our systems, contact the pharmacy to refill prescriptions, troubleshoot challenges with insurance coverage and more. Main Street’s management and training teams will equip you with the training and tools you need to perform these duties.
