Role summary

Moments Hospice is seeking a compassionate Licensed Social Worker (LSW) to provide psychosocial services to terminally ill patients and their families. This role involves psychosocial assessments, implementing care plans, providing counseling, and collaborating with interdisciplinary teams. A Master's degree in Social Work is preferred, with a minimum of 1 year of healthcare experience or 2 years of social work experience in a healthcare or social services setting. A current Social Worker license and a valid driver's license are required. Hospice experience is a plus. The position offers a competitive salary, company car, comprehensive benefits, and PTO.

Pay: $26 – $35 per hour 

Job Type: Full-time | Field-based 

Schedule: 8:30am - 5:00pm

Responsibilities:

  • Provide psychosocial support and counseling to patients and families throughout the hospice journey
  • Complete psychosocial assessments and document them in the patient's chart
  • Regularly assess and reassess the psychosocial, anticipatory grief, and pre-bereavement needs of patients and families
  • Carry out the Plan of Care, including social service goals, supportive counseling, problem solving, community referrals, and bereavement care
  • Connect families with community resources, including financial programs and eligibility support
  • Take part in weekly interdisciplinary team meetings
  • Help with funeral arrangements and provide counseling at the time of death when requested 

    • Qualifications:

  • Master's degree in Social Work preferred; Bachelor's degree in Social Work required
  • 1+ year of social work experience in a healthcare setting or social services agency
  • Current Social Worker license (required)
  • Hospice experience a plus
  • Knowledge of community resources, including state and federal financial programs and eligibility requirements
  • Understanding of hospice philosophy and the principles of death and dying
  • Strong organizational and communication skills
  • Valid driver's license and auto insurance (required)
