Role summary

Moments Hospice is seeking a PRN Hospice Social Worker to provide psychosocial services to terminally ill patients and their families. This role involves conducting assessments, developing care plans, providing counseling, and connecting patients with community resources. The position requires a current Social Worker license and a minimum of two years of social work experience in a healthcare setting. A Bachelor's degree in Social Work with 1 year of healthcare experience is acceptable, with a Master's degree preferred. Hospice experience is a plus. Pay: $26 – $35 per hour 

Job Type: Full-time | Field-based 

Schedule: 8:30am - 5:00pm

Responsibilities:

  • Provide psychosocial support and counseling to patients and families throughout the hospice journey
  • Complete psychosocial assessments and document them in the patient's chart
  • Regularly assess and reassess the psychosocial, anticipatory grief, and pre-bereavement needs of patients and families
  • Carry out the Plan of Care, including social service goals, supportive counseling, problem solving, community referrals, and bereavement care
  • Connect families with community resources, including financial programs and eligibility support
  • Take part in weekly interdisciplinary team meetings
  • Help with funeral arrangements and provide counseling at the time of death when requested 

    • Qualifications:

  • Master's degree in Social Work preferred; Bachelor's degree in Social Work required
  • 1+ year of social work experience in a healthcare setting or social services agency
  • Current Social Worker license (required)
  • Hospice experience a plus
  • Knowledge of community resources, including state and federal financial programs and eligibility requirements
  • Understanding of hospice philosophy and the principles of death and dying
  • Strong organizational and communication skills
  • Valid driver's license and auto insurance (required)
