Role summary

Remedy Home Health Care is seeking a dedicated Physical Therapist Assistant (PTA) for their West Los Angeles team. This role offers flexible scheduling (per diem, part-time, full-time) and focuses on providing one-on-one in-home care to help patients regain independence and improve quality of life. Responsibilities include delivering therapy services under a Physical Therapist's supervision, assisting with exercises and rehabilitation, monitoring progress, educating patients and caregivers, and completing documentation using WellSky Kinnser EMR. A California PTA license, CPR certification, and at least 1 year of clinical experience are required. Home health, rehab, and senior care experience are preferred.

Your Role as a Home Health PTA (Physical Therapist Assistant)

As a Physical Therapist Assistant in home health, you will deliver skilled therapy services under the supervision of a licensed Physical Therapist, focusing on rehabilitation, fall prevention, and functional mobility.

Key Responsibilities:

Provide in-home physical therapy services following the established Plan of Care

Assist patients with therapeutic exercises, mobility training, and rehabilitation programs

Support senior care patients in improving strength, balance, and independence

Monitor, document, and report patient progress to the supervising Physical Therapist

Educate patients and caregivers on home exercise programs, safety, and fall prevention

Collaborate with physicians, RNs, and the interdisciplinary care team

Complete documentation using WellSky Kinnser EMR

Qualifications:

Active California Physical Therapist Assistant (PTA) license

Current CPR certification (AHA required)

Minimum 1 year of clinical experience (home health experience preferred)

Strong interest in home health, rehab, and senior care

Excellent communication and organizational skills

Valid driver’s license, auto insurance, and reliable transportation
