Role summary

Remedy Home Health Care is seeking experienced and compassionate Registered Nurses (RNs) for per diem home health visits in the West Los Angeles and North OC areas. This role offers flexible scheduling, competitive pay, and the opportunity to provide meaningful patient care directly in patients' homes. Responsibilities include skilled nursing care, developing care plans, monitoring progress, educating patients and families, and coordinating with healthcare teams, all while adhering to home health regulations. A California RN license and at least one year of RN experience are required, with home health, geriatric, or community health experience being preferred.

Home Health RN Responsibilities

Provide skilled nursing care in patient homes, including assessments, wound care, medication management, and treatments

Develop and implement individualized home health care plans

Monitor patient progress and adjust care plans as needed

Educate patients and families on disease management, rehab, and senior care

Coordinate care with physicians, therapists, and the Director of Nursing

Document patient visits in compliance with home health regulations

Qualifications – Home Health RN Jobs

Active California Registered Nurse (RN) license (required)

Minimum 1 year RN experience (home health experience preferred)

Experience in rehab, geriatric care, or community health is a plus

Strong clinical, communication, and organizational skills

Valid driver’s license and reliable transportation

Ability to travel throughout North Orange County

Preferred Qualifications

Home Health RN experience

Public Health Nurse (PHN) certification

Experience with mobile therapy or home-based care models
