Role summary
Silverado Hospice San Diego is seeking a compassionate and skilled RN Case Manager to provide hospice and palliative care to patients and families in the Pacific Beach, Ocean Beach, La Jolla, and surrounding areas. The role involves developing individualized care plans, collaborating with an interdisciplinary team, conducting nursing assessments, and providing field supervision to LVNs and Home Health Aides. The ideal candidate must be a Registered Nurse with at least 1 year of experience, possess a valid driver's license, reliable transportation, and the ability to lift up to 50 pounds.
