Role summary
The Company is hiring a full-time Primary Care Nurse Practitioner/Physician’s Assistant (NP/PA) to provide on-site care to residents in long-term care settings. This role is ideal for a provider who values meaningful patient relationships and a collaborative care environment. Responsibilities include performing physical exams, diagnosing and treating conditions, ordering tests, prescribing medications, and collaborating with staff to develop and adjust care plans. The position requires an active and unrestricted NP, PA, or Physician license in Ohio and a minimum of 6 months of experience in long-term care or a skilled nursing facility. Strong communication, clinical, and critical thinking skills are essential, with a passion for elder care.
