Role summary
VitalCaring is seeking a compassionate and skilled Physical Therapist (PT) for a home health role. This autonomous position involves evaluating patients, developing and implementing individualized care plans, providing hands-on therapy to improve mobility and independence, and educating patients and caregivers. The role requires collaboration with a care team, accurate EMR documentation, and supervision of PTAs. Candidates must have a DPT or Master's degree, a current PT license, 1-2 years of experience (home health preferred), strong clinical assessment and critical thinking skills, excellent communication abilities, CPR certification, a valid driver's license, and reliable transportation.
