Role summary

VitalCaring is seeking a compassionate and skilled Physical Therapist (PT) to join their home health team. This autonomous, field-based role focuses on evaluating patients, developing and implementing individualized care plans, and providing hands-on therapy to improve mobility, strength, and independence in patients' homes. Responsibilities include collaborating with a care team, educating patients and caregivers, documenting progress in the EMR, and supervising Physical Therapist Assistants (PTAs). The ideal candidate has a DPT or Master's degree, a current PT license, 1-2 years of experience (home health preferred), strong clinical assessment and communication skills, and a valid driver's license.

What You’ll Do

Deliver Patient-Centered Care

  • Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
  • Develop and implement individualized care plans in collaboration with physicians and the care team
  • Provide hands-on therapy to improve mobility, strength, balance, and independence
  • Educate patients and caregivers on treatment plans, exercises, and safety strategies

    • Drive Outcomes & Recovery

  • Monitor patient progress and adjust treatment plans as needed
  • Support safe transitions of care and contribute to discharge planning
  • Identify additional patient needs and collaborate with interdisciplinary teams

    • Collaborate & Communicate

  • Partner closely with nurses, therapists, and physicians to ensure coordinated care
  • Document patient care and progress accurately and timely within the EMR
  • Participate in case conferences and team meetings

    • Lead & Support

  • Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
  • Provide guidance and education to caregivers and support staff

    • What You Bring

  • Graduate of an accredited Physical Therapy program (DPT or Master’s)
  • Current PT license in the state of practice
  • Minimum 1–2 years of experience (home health experience preferred)
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and patient education abilities
  • Current CPR certification
  • Valid driver’s license, reliable transportation, and auto insurance
