Position Details
Our Program: C-Change
C-Change works with youth who are at risk of, or have been identified as, sexually exploited minors, helping them transform their lives. The program focuses on stabilization, reducing mental health symptoms, building self-efficacy and self-determination, creating community, and connecting youth to additional resources. Our specialized model includes mental health screening, advocacy, clinical case management, and psychotherapy for at-risk youth. We serve young people up to age 21 who live throughout the Bay Area.
The Position
The ICC will work mainly with youth and young adults in C-Change. The role was is also referred to as a Mental Health Care Coordinator.
You will be responsible for assessing clients and for developing, coordinating, carrying out, and overseeing intensive, community-based behavioral health care plans. Clients have moderate to high acuity and complex needs that may be behavioral, medical, developmental, and/or psychosocial.
Each youth is supported by a Child & Family Team (CFT) that you will assemble and lead. The team draws on family, natural supports such as friends, extended family, neighbors, and community members, and formal system partners such as child welfare, schools, and juvenile probation, all working together toward the youth’s goals and toward lasting change. You will coordinate and facilitate regular CFT meetings and both monitor and contribute to the youth’s progress.
Guided by the Integrated Core Practice Model, you will use clinical judgment and discretion, in partnership with the team, to evaluate strengths and needs, choose interventions, coordinate services across disciplines, manage crises, link clients to resources, build behavioral skills, and design individualized care strategies.
The work is varied. It involves interpreting clinical information, applying professional standards, and making clinically informed decisions. You’ll provide services and attend agency meetings virtually (Zoom or phone), in the office, and in community settings such as our drop-in center, client homes, schools, and parks.
