Role summary

Western Geriatrics and Neurology, part of Brigade Health, is seeking compassionate Nurse Practitioners (NPs) or Physician Assistants (PAs) to provide mobile primary care to seniors in various settings including homes, skilled nursing facilities, and assisted living communities. The role focuses on geriatric care, chronic disease management, dementia care, and relationship-based medicine. Responsibilities include conducting mobile visits, managing acute and chronic conditions, performing assessments, care planning, collaborating with teams, and maintaining EMR documentation. A flexible schedule, guaranteed base salary plus productivity, and a full benefits package are offered. Requires an active NP or PA license and a valid driver's license with reliable transportation. Experience in primary care, geriatrics, home health, hospice, SNF, or senior care is preferred.

What You’ll Do

Provide mobile primary care visits to seniors

Manage chronic and acute conditions

Perform comprehensive assessments & care planning

Collaborate with caregivers, facilities, and interdisciplinary teams

Deliver compassionate, patient-centered care

Maintain accurate EMR documentation

Qualifications

Active NP or PA license

1+ years of primary care, geriatrics, home health, hospice, SNF, or senior care experience preferred

Strong communication and clinical skills

Valid driver’s license & reliable transportation
