Role summary

Avalon Care Center Honolulu is seeking an MDS Coordinator RN to join their team. This role is crucial for supporting quality outcomes, ensuring assessment accuracy, and helping residents receive exceptional care. The position contributes to the facility's clinical and financial success. What You'll Do

Complete resident assessments and MDS documentation
Partner with the care team to develop resident care plans
Ensure assessments are accurate, timely, and compliant
Support Medicare and Managed Care processes
Help drive quality outcomes and reimbursement accuracy

What You'll Need

Current Hawaii RN license
Previous MDS Coordinator experience
Strong knowledge of the RAI process and case management
Strong attention to detail and organizational abilities
Ability to meet all employment screening requirements
