Role summary

Home Sweet Home is seeking a licensed Physical Therapist for home health services in the Scottsdale and North Scottsdale areas. This role offers flexible scheduling and competitive per-visit compensation, with the opportunity to earn up to $103,500 annually. Responsibilities include conducting in-home evaluations, developing treatment plans to improve patient mobility and independence, educating patients and caregivers, and documenting visits using WellSky Kinnser EMR. A minimum of 1 year of clinical experience is required, with home health experience preferred. Candidates must possess an active Arizona Physical Therapist license, CPR certification, reliable transportation, and strong communication skills.

Your Role: Home Health Physical Therapist

You will provide one-on-one care in patients’ homes, helping them regain strength, mobility, and confidence in their daily lives.

Key Responsibilities

Conduct in-home evaluations, start of care, and follow-up visits

Develop and implement individualized treatment plans

Improve strength, balance, mobility, and functional independence

Educate patients and caregivers on safety and equipment use

Document visits using WellSky Kinnser EMR

Collaborate with physicians, RNs, and the care team

Qualifications

Active Arizona Physical Therapist license

CPR certification (AHA)

Minimum 1 year of clinical experience (home health preferred)

Strong clinical judgment and time management

Reliable transportation, valid driver’s license, and insurance

Strong communication and a patient-first approach
