Role summary

Health Link Home Health & Hospice is seeking a compassionate and organized Registered Nurse (RN) to join their South Bay team. This full-time role offers autonomy and support for nurses, with a competitive compensation structure, AI-assisted charting, and real-time clinical support. Responsibilities include managing caseloads, performing nursing assessments (OASIS), administering medications, providing wound care and IV therapies, coordinating with physicians and the interdisciplinary team, educating patients and families, and maintaining accurate clinical notes. A current California RN License and a minimum of 1 year of professional nursing experience are required. Home Health experience is preferred.

Your Role

Manage Your Caseload: Perform comprehensive nursing assessments (OASIS), including Start of Care, Recertifications, and Discharges.

Skilled Care: Administer medications, provide wound care (including Wound Vacs), and manage IV therapies.

Coordinate: Act as the primary point of contact for physicians and the interdisciplinary team (PT/OT/ST) to ensure the care plan is working.

Educate: Empower patients and families to manage their conditions safely at home.

Document: Maintain accurate, timely clinical notes on company-issued iPads.

Requirements

LICENSE: Current, active California RN License is REQUIRED to apply. Applications without a valid CA license number will not be reviewed.

Experience: Minimum 1 year of professional nursing experience required. Home Health experience is preferred, but we are willing to train strong hospital/SNF nurses.

Travel: Valid California Driver’s License and reliable transportation are mandatory.
