Home Sweet Home is seeking a licensed Physical Therapist (PT) for a Home Health role in the West Valley of Phoenix, serving areas like Glendale, Peoria, and Surprise. This position offers flexible scheduling, competitive per-visit compensation (up to $103,500 annually), and a supportive team environment. The PT will provide one-on-one care in patients' homes, focusing on improving mobility, strength, balance, and functional independence. Responsibilities include conducting evaluations, developing treatment plans, educating patients and caregivers, and documenting visits using WellSky Kinnser EMR. A minimum of 1 year of clinical experience is required, with home health experience preferred. A valid Arizona PT license, CPR certification, reliable transportation, and strong communication skills are essential.

Compensation: Per Visit | Earn up to $103,500 annually based on productivity

Sign-On Bonus: $5,000 for Full Time

Your Role: Home Health Physical Therapist

You will provide one-on-one care in patients’ homes, helping them regain strength, mobility, and confidence in their daily lives.

Key Responsibilities

Conduct in-home evaluations, start of care, and follow-up visits

Develop and implement individualized treatment plans

Improve strength, balance, mobility, and functional independence

Educate patients and caregivers on safety and equipment use

Document visits using WellSky Kinnser EMR

Collaborate with physicians, RNs, and the care team

Qualifications

Active Arizona Physical Therapist license

CPR certification (AHA)

Minimum 1 year of clinical experience (home health preferred)

Strong clinical judgment and time management

Reliable transportation, valid driver’s license, and insurance

Strong communication and a patient-first approach
