Role summary
Hopscotch Primary Care is seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to provide comprehensive, interdisciplinary primary care. The role focuses on patient outcomes, quality metrics, and patient experience, managing a panel of approximately 15 patients daily with dedicated support staff. Responsibilities include office and telehealth visits, annual wellness assessments, and working within a continuous improvement framework. Candidates must have a minimum of three years' experience in primary care specialties and experience with HCC documentation, ICD-10 coding, and HRAs.
