Role summary

VitalCaring is seeking a compassionate and skilled Physical Therapist (PT) to join their home health team. This autonomous, field-based role focuses on evaluating patients, developing and implementing individualized care plans, and providing hands-on therapy to improve mobility, strength, and independence in patients' homes. The PT will collaborate with physicians and the care team, supervise PTAs, and educate patients and caregivers. A DPT or Master's degree, state PT license, 1-2 years of experience, strong clinical assessment skills, excellent communication, and CPR certification are required. Home health experience is preferred. The role offers flexible scheduling, autonomy, and opportunities for career advancement.

What You’ll Do

Deliver Patient-Centered Care

  • Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
  • Develop and implement individualized care plans in collaboration with physicians and the care team
  • Provide hands-on therapy to improve mobility, strength, balance, and independence
  • Educate patients and caregivers on treatment plans, exercises, and safety strategies

    • Drive Outcomes & Recovery

  • Monitor patient progress and adjust treatment plans as needed
  • Support safe transitions of care and contribute to discharge planning
  • Identify additional patient needs and collaborate with interdisciplinary teams

    • Collaborate & Communicate

  • Partner closely with nurses, therapists, and physicians to ensure coordinated care
  • Document patient care and progress accurately and timely within the EMR
  • Participate in case conferences and team meetings

    • Lead & Support

  • Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
  • Provide guidance and education to caregivers and support staff

    • What You Bring

  • Graduate of an accredited Physical Therapy program (DPT or Master’s)
  • Current PT license in the state of practice
  • Minimum 1–2 years of experience (home health experience preferred)
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and patient education abilities
  • Current CPR certification
  • Valid driver’s license, reliable transportation, and auto insurance
