Role summary

Wremlex Health Care is seeking a full-time Program Manager to oversee operations within the Intellectual and Developmental Disabilities (ID/DD) services domain. This onsite role requires the ability to schedule, train, supervise, and evaluate staff, ensuring program compliance with state and federal regulations (DBHDS, DMAS, CMS). The Program Manager will be responsible for assessing service recipient needs, developing and monitoring individual service plans, and maintaining all necessary documentation. Key responsibilities include staff hiring and scheduling, direct care support when needed, and fostering positive relationships within the community and with stakeholders. Proficiency in computer skills and strong communication abilities are essential for this role, which demands a minimum of 2 years of supervisory experience in the ID/DD field.

Monday through Friday 9am to 5pm, with occasional on-call hours

  • Schedule, train, supervise, and evaluate assigned employees.
  • Assess the needs of service recipients through referrals and interviews.
  • Assure program compliance with applicable rules, regulations, and laws including those made by DBHDS Licensing, the Department of Medical Assistance Services and the Centers of Medicare and Medicaid. Assure the quality of support to service recipients.
  • Involvement in the hiring process of DSPs to work with service recipients
  • Schedule assigned employees to meet program needs.
  • Communicate any identified risk or issue with management for quick action.
  • Communicate effectively with Person Centered Planning Team Members.
  • Maintain positive, professional relationships with service recipients, co-workers, supervisors, families, community members and outside agencies.
  • Develop, implement and monitor individual service plans in collaboration with the service recipients, their families, and other service providers.
  • Outreach and marketing program to CSBs.
  • Facilitate service recipients choice regarding services and supports.
  • Ensure individuals privacy, dignity, and respect and freedom form coercion and restraint.
  • Prepare and maintain written and electronic documentation of individual service plans, phone contacts, DBHDS requirements, and other information to meet program, agency, and licensure requirements.
  • Train DSP to document and oversee input of service and other documentation to the electronic records system.
  • Maintain individuals' confidential folder in accordance with regulations and policies.
  • Provide one-to-one training for individuals with ID/DD and direct care of individuals.
  • Provide coverage for individuals when needed.
  • Perform other duties as required.

KNOWLEDGE, SKILLS and ABILITIES:

  • Ability to supervise other staff or at least 2 years' experience in the field of ID/DD providing direction, development and implementation, direct supervision and monitoring (observation and evaluation of staff implementing care, service plans & interacting with clients) to the service provided.
  • Ability to act as a representative of Wremlex Health Care in the community.
  • Ability to effectively communicate with individuals of diverse backgrounds in the workplace and in the workplace.
  • Experience developing, conducting, and approving assessments and individual service plans or treatment plans to meet the needs of the individuals serviced.
  • Completion of acceptable criminal background checks and TB health screening.
  • Proficient computer skills.
  • Ability to assess individuals' needs.
  • Ability to meet licensure requirements.
  • Able to communicate clearly and effectively (verbally and written) with individuals, all levels of personnel, internal and external to the agency.
  • Ability to plan, organize and carry out complex tasks given verbal and/or written instructions and guidelines.
  • Other duties as assigned.

Job Types: Full-time, Contract

Pay: $75,000.00 - $78,000.00 per year
